Intensive Case Manager

Organizational Overview

Prevention Point Philadelphia (PPP) is a multi-service public health organization dedicated to promoting health, empowerment, and safety for communities affected by drug use and poverty. PPP meets drug users where they are, providing information and access to resources with dignity and respect. Using a harm reduction approach, PPP offers low threshold, culturally sensitive, non-judgmental services to address the health and social service needs of drug and hormone users and sex workers in Philadelphia. PPP promotes harm reduction through sterile syringe exchange, mobile medical care, referrals to social services, and prevention case management services.

Program Overview

The Intensive Case Manager (ICM) is part of PPP's Medical Case Management Program (MCM), providing high-touch, community-based case management to people living with HIV who experience significant barriers to accessing medical and behavioral health services in traditional care settings. Through low-barrier, wraparound care and advocacy across internal and external health systems, the program supports participants in addressing complex medical needs and social determinants of health.

MCM is integrated into Prevention Point’s Sana Clinic, a one-stopshop care setting that brings together HIV care, primary care, and medications for opioid use disorder (MOUD). This integrated model allows participants to access medical, behavioral health, and social services in one location while receiving individualized support from their case manager. The program is funded through the Department of HIV Health.

Position Overview

The ICM provides trauma-informed, participant-centered care to individuals living with HIV who often experience complex medical, behavioral health, substance use, housing, and/or psychosocial needs. Taking a harm reduction approach, the ICM meets participants where they are, developing and implementing individualized care plans that address their needs and incorporate their goals. The role includes connecting participants to internal and external services and resources, coordinating and monitoring care, and supporting crisis de-escalation and problem-solving to help participants achieve their goals.

Position Duties & Responsibilities

• Conduct routine assessments, including benefits eligibility and enrollment, review of medical and behavioral health histories, coordination of integrated and specialty care service
• Provide escort, advocacy, and in-person support at internal and external physical and behavioral health appointments.
• Conduct participant interviews to assess medical and psychosocial needs. Use these assessments to inform and create collaborative care plans and goals.
• Link clients to appropriate medical, mental health, substance use, and general support services
• Coordinate care with external providers, hospitals, social workers, and treatment centers for individuals both inpatient and outpatient, assisting with patient advocacy and care plans.
• Oversee care delivery and monitor ongoing care.
• Conduct Level of Care Assessments for clients seeking MOUD treatment referrals.
• Manage a caseload of 25-30, with varying levels of need.
• Assist in discharge planning from hospital or treatment as needed.
• Collaborate with team members and other PPP departments such as STEP, TALC, HCV, Housing, Outreach, Drop-In, and Syringe Services to ensure continuity of care for participants engaged in multiple PPP programs.
• Assist with crisis de-escalation.
• Conduct outreach for participants and meet with them face-to-face, in the community and around PPP.
• Meet program specific documentation and data entry requirements, as discussed with immediate supervisor, by deadlines.
• Actively participate in supervision, team, and staff meetings.
• Work in a fast-paced environment and assist with additional organizational needs.
• Perform other duties as assigned by supervisor.

Qualifications

• Belief in promoting the health and dignity of individuals who use drugs through a harm-reduction lens.
• Bachelor’s degree in public health, social work, psychology, or a related field preferred but not required.
• A minimum of 1 year of experience working in the field of case management, outreach, or other related public health field.
• At least two years’ experience with communities impacted by homelessness, poverty, substance use, and/or HIV/AIDS.
• Experience with working with individuals experiencing homelessness impacted by substance use, mental health, and/or complex medical issues.
• Skill in communicating effectively with a variety of people of different backgrounds
• Strong computer and professional writing skills.
• Ability to use a harm reduction approach to housing and substance use.
• Dynamic, creative, flexible, able to learn and adapt in a changing environment
• Ability to work independently and as part of a team.
• Ability to navigate public transportation and work outdoors.

Minimum Qualifications

• Skilled in communicating effectively with a variety of people of different backgrounds.
• Strong computer and professional writing skills.
• Ability to use a harm reduction approach with people who use drugs and engage in sex work.
• Dynamic, creative, flexible, able to learn and adapt to a changing environment.
• Ability to work independently and as part of a team.
• Commitment to a harm reduction philosophy including meeting people where they are and creating a safe, non-judgmental environment.

Other Requirements

• This position includes the normal physical demands of a community-based health program, including lifting/moving up to 15lbs, bending, and carrying. Other physical requirements include walking and standing for long periods and sitting for up to 30 minutes
• This position requires outreach and a level of comfort taking public transportation with a participant.
• This position requires travel to outreach locations within Philadelphia and the ability to work out-of-doors for up to 3 hours
• This position is exempt from the overtime provisions of the Fair Labor Standards Act
