    Carter Burden Network (CBN) – Geriatric Community Transition Program

    Help older New Yorkers return home safely after a hospital stay—and make a meaningful impact every single day.

    Carter Burden Network, in partnership with Lenox Hill Hospital–Northwell Health, is launching a new pilot program supporting adults 60+ as they transition from hospital or emergency department discharge back to their homes. We are seeking compassionate, detail-oriented Community Care Navigators to join our Health & Wellness Team and strengthen the safety net for older adults across Upper Manhattan and Roosevelt Island.

    What You’ll Do

    As a Community Care Navigator, you’ll play a hands-on role in stabilizing the critical first weeks after discharge by:

    Hospital & Clinical Engagement

      • Meeting patients, caregivers, and hospital discharge teams onsite
      • Coordinating communication between hospital staff, families, and community providers

    In-Home Assessments

      • Conducting structured home visits
      • Identifying risks, medication access issues, equipment needs, and home safety concerns
      • Providing referrals to strengthen the home environment

    Patient Education & Self-Management

      • Explaining discharge instructions in clear, culturally responsive ways
      • Supporting symptom monitoring and disease self-management
      • Ensuring clients understand their medications

    Care Coordination & Benefits Navigation

      • Scheduling follow-up appointments and confirming provider connections
      • Linking clients to transportation, home care, nutrition programs, Medicare/Medicaid support, legal aid, and mental health services

    Documentation & Teamwork

      • Maintaining accurate records in the program database
      • Participating in trainings, meetings, and case conferences
      • Reporting urgent concerns promptly

    What You Bring

      • Bachelor’s degree (BA/BS) required
      • Experience in social services, care coordination, discharge planning, or community health preferred
      • Strong communication skills, cultural sensitivity, and comfort working in dynamic environments
      • Ability to travel throughout Upper Manhattan and Roosevelt Island for home visits
      • Bilingual proficiency is considered an asset, particularly in languages reflective of the communities we serve. Preference will be given to candidates who ae proficient in a second language, with Spanish proficiency especially valued.

    This part-time position is scheduled for 21 hours per week, Monday through Friday, generally within the hours of 9:00 a.m. to 5:00 p.m. The role involves a blend of on-site responsibilities at the Leonard Covello Older Adult Center and Lenox Hill Hospital, along with community-based and in-home visits as needed to support clients throughout the transition process. During scheduled work hours, employees are expected to remain accessible by phone and email to ensure timely communication and coordination of care.

    Compensation & Benefits

    The position offers an hourly rate of $27, fully aligned with the NYC Pay Transparency Act. Employees in this position are eligible for CBN’s comprehensive benefits package, which includes multiple health and dental plan options—some of which are 100% employer‑paid—along with 15 vacation days, 4 personal days, 10 sick days, and paid holidays. The organization also provides a 403(b) retirement plan featuring a 3% employer contribution, with eligibility for an additional matched contribution after one year of service.
