    Description

    Position Summary:

    The Case Management Specialist provides direct student support services in alignment with Mercy Learning Center’s mission to provide holistic education and support services that empower women to achieve self-sufficiency and reach their full potential. This role delivers culturally responsive, strengthsbased case management services to adult learners, assisting students in navigating systems and accessing resources that support their educational and personal goals.

    The Case Management Specialist works collaboratively with instructional and administrative staff to ensure coordinated and respectful student support.

    Job Specific Responsibilities

    • Complete initial student intake assessments, profiles, and budgeting tools.
    • Develop individualized support plans in collaboration with students.
    • Maintain accurate, timely, and confidential case notes and documentation.
    • Keep detailed records of cases, referrals, and agency contacts.
    • Provide referrals to appropriate community agencies and services.
    • Advocate for students as appropriate when interacting with external agencies.
    • Support students with available community services and benefits applications.
    • Provide guidance regarding immigration resources and referrals as appropriate.
    • Coordinate referrals to appropriate internal staff and resources, as needed, to support students’ academic, career, and personal goals.
    • Assist with organization and distribution of approved donations to students.
    • Maintain updated informational materials and resource boards.
    • Collaborate with the Manager of Case Management and Manager of Community Partnerships and Resources to review cases and ensure quality service delivery.
    • Participate in student registration.
    • Attend scheduled staff meetings and professional development activities.
    • Submit required case documentation and reports to the Manager of Case Management.

    Qualifications

    • Bachelor’s degree in Human Services or related field preferred.
    • Minimum two (2) years experience in case management or social services.
    • Fluency in Spanish and English – strong oral and written skills required.
    • Demonstrated ability to provide comprehensive and intensive services.
    • Strong interpersonal, problem-solving, and organizational skills.
    • Ability to work both independently and as part of a team.
    • Excellent computer and documentation skills.
    • Valid driver’s license and access to reliable transportation with proof of insurance.

    Physical Demands and Work Environment

    • Ability to lift and carry up to twenty (20) pounds as needed.
    • Ability to sit or stand for extended periods.
    • Occasional off-site meetings or appointments may be required.

    Work Schedule

    Monday - Friday; 9am to 5pm
