    Case Manager — Supportive Housing & Community Services

    East Central Illinois (Hybrid)

    $43,650 – $44,590 per year (based on experience)

    Full-Time | 35-Hour Work Week | Flexible Schedule

    Are you passionate about helping individuals and families living with HIV/AIDS achieve housing stability, independence, and improved quality of life? The Greater Community AIDS Project (GCAP) is seeking a compassionate, motivated Case Manager to support clients transitioning into permanent supportive housing while connecting m them to essential healthcare, employment, and community resources.

    What You’ll Do:

    • Client Engagement & Goal Planning
    • Case Management & Advocacy
    • Documentation & Compliance
    • Outreach & Support

    What You Have

    • Motivational interviewing and strengths-based case management experience
    • Trauma-informed care approach
    • Strong communication, organization, and documentation skills
    • Crisis de-escalation ability
    • Valid driver’s license, reliable transportation, clean driving record, and willingness to transport clients
    • Bachelor’s degree in social work, Human Services, Psychology, Sociology, or related field preferred OR Bachelor’s degree in another field with at least 20 semester hours in human services/social services
