    Job Summary

    The Program Community Manager connects patients with A1C screenings, diabetes education, resources, appointments, outreach services, and ongoing support by connecting at community sites and by telephone. By providing compassionate, practical, and culturally responsive support to individuals and families you are offering help, hope and better health to people in need. Additionally, the Community Manager will work with partners, provide direct patient outreach service and collect data. The ideal candidate is bilingual, organized, dependable, detail-oriented, and committed to improving access to diabetes-related services in the community.

    Key Responsibilities

    • Provide in-person community-based services, education, and support to participants. 
    • Manage relationships with community organizations, healthcare providers, food pantries, senior centers and other community partners.
    • Manage, coordinate, and support volunteers during community events and outreach activities. 
    • Assist individuals to access diabetes education, community resources, Pathway to care services, and other Diabetes Foundation programs.
    • Support outreach efforts, workshops, events, and community presentations related to diabetes awareness, prevention, and management.
    • Help schedule appointments, follow up with participants, and maintain accurate records of services provided.
    • Collect, organize, and track required data from each partner organization to support reporting, program evaluation, and grant or foundation requirements.
    • Communicate with partner organizations to request information, confirm data accuracy to ensure timely submission of Diabetes Foundation reports or updates.

    Qualifications

    • Bilingual fluency in English and Spanish.
    • A valid driver's license is required.
    • Strong communication, listening, and interpersonal skills.
    • Experience in community outreach, customer service, and social services preferred.
    • Ability to work respectfully with diverse populations and maintain a welcoming environment.
    • Proficiency with Microsoft office (Word, Excel, PowerPoint, Outlook), spreadsheets, Canva, email, and data entry.
    • Strong organizational skills and attention to detail.
    • Ability to work independently and as part of a team.
    • Knowledge of diabetes, chronic disease support, or community health resources is helpful but not required.

    Work Schedule

    This is a full-time hourly position, scheduled for 40 hours per week. Standard work hours are Monday through Friday from 9:00 a.m. to 5:00 p.m. Occasional flexibility may be requested for community events, outreach activities, or special programs.

    Level of Language Proficiency

    Bilingual fluency in English and Spanish preferred.
