    POSITION SUMMARY: The Case Manager provides support to a community of 55+ aged residents in a 41-unit East Boston based affordable housing site. The position requires a motivated and resourceful individual who is sensitive to resident needs; has excellent service coordination, systems navigation, and case management skills; and can thrive in a relatively independent environment working with a population affected by trauma and with diverse physical disability and mental health needs.

    PRIMARY RESPONSIBILITIES:

    • Provide direct services such as case management, information and referrals, and translation assistance
    • Conduct intake assessments with residents, discussing options of services referrals and eligible programs that best fit their needs
    • Support residents in completing and tracking progress in public assistance applications for rent/utility assistance, food access, supplemental income programs, and others
    • Strengthen referral connections with local government and social service agencies
    • Serve as an advocate and liaison for residents with third parties (e.g. service agencies, property management, government agencies, etc.)
    • Maintain case management records in CiviCRM, ensuring consistent and precise record keeping of case referral processes and ongoing updates
    • Hosting coffee hours, small events, or other get-to-know resident activities

    MINIMUM QUALIFICATIONS:

    • Intermediate/fluent Spanish language skills strongly preferred
    • Combination of education and/or experience in providing case management services, coordinating community resources, or other supportive care

    REQUIRED SKILLS:

    • Ability to work with people from ethnically and socio-economically diverse backgrounds
    • Sensitivity to individuals facing challenges related to mental health, substance abuse, former homelessness, disability, poverty, and other traumas
    • Remain composed in stressful situations and exercising crisis de-escalation and resolution skills where appropriate
    • Maintain confidentiality of sensitive case information
    • Capacity to operate independently, taking initiative and exercising own judgment for majority of work
    • Ability to navigate community resources and services, with particular emphasis on senior-aged populations
    • Strong organizational skills
    • Computer skills in Microsoft suite (i.e. Outlook, Word, Excel)

    • Must be able to get to housing communities in East Boston where services are provided
    • Ability to travel throughout the building and attend meetings throughout the community
    • Ability to handle a significant level of stress
    • Ability to set up and clean up facilities for events with occasional lifting and pushing
