    Description

    Position: Traditional Health Worker

    Reports to: Care Team Manager

    Hours: 40 hours/week

    Position Summary: This position provides on-site support to guests and residents of Unity Shelter

    programs. This position provides direct assistance and advocacy to individuals seeking primary, dental,

    mental health, and vision care, as well as safe, permanent housing.

    Location: Multiple locations, Corvallis OR

    Position Responsibilities:

    • Provide on-site support for guests or residents at assigned location(s)
    • Build strong and professional rapport with guests
    • Assist case managers and guests in determining goals and outlining plans for success
    • Accompany guests to housing, physical, mental health, and other appointments as necessary
    • Assist in advocacy for client needs
    • Assist in culturally sensitive resource navigation and application processes
    • Improve and facilitate access to and utilization of resources and programs within Unity Shelter (US) and the greater community
    • Collaborate with Unity Shelter staff and leadership in program planning and implementation
    • Complete and stay up-to-date on US's required training
    • Complete and maintain accurate notes and data in US databases in a timely and professional manner
    • Other duties as assigned

    Required Skills and Experience:

    • Community Health Worker certificate required (Unity Shelter could cover this training for the right candidate)
    • Valid driver’s license and reliable transportation
    • Lived experience with homelessness, addiction, mental health, gender identity, or other relevant lived experience
    • Familiarity with, and ability to, apply trauma-informed approaches in client interactions
    • Commitment to equity and inclusion
    • Ability to multitask and stay organized
    • Ability to work independently
    • Ability to communicate openly and effectively with participants, staff, and community partners
    • Experience using Microsoft and Google applications
    • Strong written and verbal communication skills
    • Consistent, timely, and appropriate communication style
    • Knowledge of resources available in Corvallis, Benton County, and surrounding areas preferred
    • Knowledge and experience with Trauma-Informed Care, Mental Health First Aid, homelessness, cultural competence, and cultural humility preferred
