About Health-Related Social Needs & Health Care Partnerships
As a partner in MassHealth's Health-Related Social Needs program, Project Bread is treating food insecurity through the healthcare system to improve long-term patient health outcomes. This revolutionary program allows Medicaid dollars to be used on “non-medical” expenses to address two social determinants of health — food insecurity and housing stability.
One of the only kinds in the nation, Project Bread’s Health Care Partnerships (HCP) goes beyond prescriptions for food by offering comprehensive case management to certain eligible MassHealth patients who are both food insecure and have a physical or behavioral health diagnosis.

Starting Salary Range: $72,000 - $76,000
The Role: The Senior Program Manager is the operational backbone of the Health Care Partnerships (HCP) program, ensuring program members reliably receive food benefits, cooking classes, counseling/ nutrition education and kitchen supplies to support their health. This role manages the staff, systems, and vendor relationships that make our program delivery possible, and works closely with the Director and Vice President to strengthen program infrastructure.
The ideal candidate is systems-minded, loves problem solving, and is energized by helping implement a high-impact program. They bring strong experience supervising a team and managing vendor relationships. They work fluently in Excel and Salesforce and can pick up new technologies quickly and confidently as program needs evolve. This role is for someone who finds meaning in building the behind-the-scenes operations that allows a mission-driven program to deliver on its promises to the people it serves.

Work Schedule
All Project Bread employees currently work a hybrid, remote and in-person, model and are expected to be present in the East Boston office on a regular basis. Remote work schedules are determined by departments and can vary across teams. The Senior Manager is required to work from the office twice a week, including the full-team workday on Wednesdays. Hybrid schedules can shift based on role and team needs.
This position requires occasional work during non-traditional hours. This includes weekends, when a month starts on a Saturday or Sunday and occasional availability until 6:30pm for cooking classes.
