Position Summary
Reporting to the Program Director, the Caseworker provides individualized support and case management for young adults participating in Bridge Builder Communities’ programs. As part of a trauma-informed, wraparound care team, the Caseworker helps residents set and achieve educational, employment, and life goals that lead to long-term stability and independence.
This position is responsible for conducting assessments, developing individualized development plans, coordinating services, facilitating life skills and job readiness sessions, and monitoring client progress toward self-sufficiency. The Caseworker works closely with the Program Director, Resident Advisor, and external partners to ensure residents receive comprehensive, coordinated, and high-quality support. The role requires professionalism, empathy, and strong organizational and communication skills.
Responsibilities
Client Assessment & Planning
- Conduct comprehensive assessments to identify each client’s strengths, needs, barriers, and goals.
- Develop and maintain individualized development plans focused on education, employment, financial stability, and life skills.
- Collaborate with clients to establish short- and long-term objectives and identify necessary supports and milestones.
Case Management & Client Support
- Provide ongoing coaching and mentorship to help clients navigate challenges, build confidence, and achieve progress.
- Coordinate access to supportive services such as healthcare, transportation, financial literacy, and workforce training.
- Facilitate or co-facilitate workshops and group sessions on topics such as job readiness, budgeting, and life skills.
- Maintain timely, accurate documentation of all client interactions, plans, and outcomes in the case management system.
- Provide crisis intervention, referrals, and follow-up support as needed.
Collaboration & Communication
- Work closely with the Program Director to ensure case management practices align with program goals and outcomes.
- Partner with community organizations, schools, and employers to identify opportunities for education, training, and employment.
- Communicate regularly with the Community Advisor regarding resident conduct, engagement, and after-hours needs.
- Participate in team meetings, case reviews, and staff trainings to ensure consistency in service delivery and continuous learning.
Program Implementation & Reporting
- Support the implementation of BBC’s trauma-informed and person-centered program model.
- Track and report client progress, successes, and challenges to the Program Director.
- Contribute to the preparation of impact stories and outcome data for funder and organizational reporting.
- Stay informed about community resources, best practices, and local trends affecting young adults transitioning to independence.

Qualifications
- Bachelor’s degree in social work, counseling, psychology, education, or a related field required; Master’s degree preferred.
- Experience: Minimum of 3–5 years of experience providing case management, coaching, or counseling within a human services, housing, or workforce development setting.
- Knowledge of trauma-informed care and best practices in supporting young adults transitioning to independence.
- Strong familiarity with community resources related to education, employment, housing, and supportive services.
- Excellent communication, documentation, and interpersonal skills with diverse populations.
- Demonstrated ability to work both independently and collaboratively as part of a multidisciplinary team.
- Proficiency in Microsoft Office and case management systems preferred.
