    General Info on SAGE:

    SAGE is the country’s largest and oldest organization dedicated to improving the lives of lesbian, gay, bisexual and transgender (LGBTQ+) older people. Founded in 1978 and headquartered in New York City, SAGE is a national organization that offers supportive services and consumer resources to LGBTQ+ older people and their caregivers, advocates for public policy changes that address the needs of LGBTQ+ older people, provides education and technical assistance for aging providers and LGBTQ+ organizations through its National Resource Center on LGBTQ+ Aging, and cultural competence training through SAGECare. SAGE supports a growing network of 70+ SAGECollab Partners across the country.

    Who We Need:

    The South Florida Care Manager assists and provides services to LGBTQ+ older adults in Broward County as described below. This position will report to the Assistant Director of South Florida Programs.

    What You'll Do:

    • Open cases and provide case management, information, referrals, and advocacy to LGBTQ+ people over age 60 in Broward County (and over age 50 for participants living with HIV, participants who are veterans, and transgender participants)
    • Conduct outreach throughout Broward County to attract new participants and spread awareness about SAGE services
    • Perform new participant intakes, including administration of mental health screenings
    • Respond to walk-ins and provide a presence and back-up to the South Florida Friendly Visitor Program Coordinator
    • Perform home visits to assess and provide ongoing support to participants
    • Conduct benefit and entitlement eligibility assessments, and assist clients with public benefits applications and other entitlement programs and opportunities
    • Lead or co-lead support groups and/or other workshop activities
    • Develop innovative, Person-Centered Trauma-Informed (PCTI) programming
    • Collaborate with the South Florida Friendly Visitor Coordinator, providing support related to client cases
    • Collect and enter all participant service unit data and client notes in database(s) thoroughly and in a timely manner (by the end of each day), in accordance with organizational policies and regulatory mandates, as well as fulfilling all other reporting requirements
    • Develop good relationships with external providers to facilitate connection with other community resources and providers
    • Represent SAGE at community events, conducting outreach and/or presenting workshops
    • Perform other duties as needed or assigned

    While no one person embodies all the qualities outlined below, the ideal candidate will possess many of the following professional and personal abilities, attributes, and experiences. Please consider applying, even if you do not have *all* the “need-to-haves!”

    Need-to-haves:

    • BSW or MSW, or sufficient professional experience in case management and/or with LGBTQ+ older adults
    • Strong organizational skills with the ability to manage and prioritize multiple tasks
    • Strong written and verbal communication skills
    • Flexible and dependable – this position requires a seven-hour workday, with an additional one hour for lunch; some occasional evening or weekend hours may be required with advance notice
    • Ability to work independently as well as an involved member of the South Florida team
    • Comfortable working in a fast-paced environment
    • Ability to organize files, reports, schedules, dates and information
    • Self-directed and able to work with light supervision
    • Computer literate, with knowledge of Microsoft Office and ability to learn other pertinent software systems and data bases
    • Willingness/ability to travel throughout Broward County and surrounding areas

    Nice-to-haves:

    • Familiarity with LGBTQ+ culture and communities
    • Experience working with aging populations and the public benefits associated with them
    • Familiarity with issues of cultural and linguistic competency with regards to ability, race, ethnicity, class, sexuality, gender identity and expression, and other characteristics that affect the lives of diverse older adults
    • Familiarity with social, medical, and aging networks
    • Proficiency in Spanish

    What We Offer:

    • The status: This is a permanent, full-time, non-exempt position
    • The union affiliation: This is a union-covered position, and the successful candidate will be required to join the SAGE Workers Union and is covered by the Union’s Collective Bargaining Agreement.
    • The location and work schedule: ~35 hours per week, M-F 9:30-5:30pm Occasional schedule adjustments may also be necessary, per program necessity
    • The salary: $60,000; or $65,000 for candidates who possess a master's degree in a related field (Note: salary is in accordance with the Collective Bargaining Agreement, which places each union-covered role into a specific salary band with set compensation determined by years of service in that salary band at SAGE)
