Ohel is seeking a Licensed Mental Health Clinician to provide intensive short-term case management for individuals with Severe Mental Illness that are being discharged following a psychiatric hospitalization. The program supports individuals by assisting them in their transition back into the community. Working on an innovative field-based team, this role will include following up on referrals by contacting hospital social workers, engaging and building rapport with participants while still hospitalized, facilitating optimal discharge plan, ensuring that participants maintain linkages to relevant community providers and resources, offering counseling and skill development.
Responsibilities:
- Engage in transition planning and needs assessment
- Monitor discharge with hospital social workers
- Build and maintain collaborative relationships with community providers
- Facilitate and ensure follow-up with treatment providers
- Provide short-term counseling and risk assessment
- Provide crisis intervention and support services
- Resolve clinical issues affecting participants ability to maintain tenure in the community
- Assist with appointment navigation and referrals
- Accompany clients to and from appointments
- Assist participant in identifying treatment goals and monitor participant progress and goal attainment
- Conduct family conferences and psychoeducation
- Keep detailed and comprehensive records of all contact with clients and collaterals
Qualifications:
- Masters degree in Social Work or Mental Health Counseling or a related field required (LMSW, LCSW, LMHC)
- Knowledge of mental illness, substance use disorders, and NYC behavioral health programs
- Knowledge of treatment and rehabilitation programs
- Knowledge of techniques for identifying and preventing potentially violent behavior, including crisis management techniques
- Ability to develop, evaluate, implement and modify a clinical treatment intervention to meet the needs of individual recipients
The ideal candidate is a self-starter who can work independently as well as within a team and has excellent follow up skills. This full-time position is located in Brooklyn. Hybrid schedule offered, with inpatient, home and community visits required. Position includes a competitive benefits package.
