    For more than 20 years, Fred's Footsteps has provided financial assistance to working families facing the overwhelming challenges of a child's serious illness, injury, or disability. By partnering with hospital social workers throughout the Philadelphia region, Fred's Footsteps helps families maintain financial stability during one of the most difficult times in their lives.

    Fred’s Footsteps is seeking a compassionate, highly organized, and detail-oriented Program & Family Services Coordinator to join our small, dynamic team. Reporting directly to the Program Manager, this role sits at the heart of our mission, blending clinical insight with structured grant administration. This position is ideal for a master's-level social worker who is passionate about supporting families, building relationships with healthcare professionals, and ensuring a seamless grant process from referrals through follow-up. The Program & Family Services Coordinator will coordinate grant application activities; provide direct support to hospital social workers and accepted families; maintain program data; and assist with outreach and program development.

    Reports to: Program Manager

    Hours: 40 hour hybrid work week to be fulfilled in the Conshohocken office and remotely.

    Salary Range: $65,000 - $78,000

    Core Responsibilities:

    Social Worker Support

      • Serves as a primary point of contact for hospital social workers throughout the grant application process.
      • Provides information, guidance, and support regarding eligibility, documentation requirements, and application completion.
      • Communicates with referring social workers in a compassionate, timely, and professional manner.
      • Maintains confidentiality and ensures all applicant information is handled with sensitivity and professionalism.

    Program Administration

      • Review grant applications for completeness and accuracy.
      • Coordinate the collection of required documentation and communicate with referring social workers regarding required information.
      • Prepare application materials for Grant Advisory Committee review.
      • Document grant decisions and communicate outcomes to referring social workers.
      • Assist with grant payments as requested and maintain accurate program records.
      • Assist with grant follow-up activities, including collecting outcomes, impact stories, and family feedback.
      • Maintain electronic case files and ensure data integrity within program databases.
      • Compile program statistics and assist with reports for leadership, the Board of Directors, and funders.

    Accepted Family Support

      • Welcomes and orients families to the Fred’s Footsteps Program.
      • Assists families in navigating available resources and identifying appropriate referrals when additional needs are identified.
      • Serve as point person and ongoing resource for families throughout their year in the program.
      • Maintain confidentiality and ensure all family information is handled with sensitivity and professionalism.

    Outreach and Relationship Building

      • Create and facilitate family advisory board to build connection with former families served.
      • This group is envisioned to provide input into matters that impact our programs and services
      • Help cultivate strong relationships with hospital social workers and healthcare partners.
      • Assist with presentations, outreach activities, and educational meetings with existing and prospective referral partners.
      • Represent Fred's Footsteps at community events, conferences, and fundraising activities as requested.

    Program Quality and Development / Data

      • Identify opportunities to improve program efficiency and participant experience.
      • Support quality assurance efforts by monitoring program processes, administering family surveys, and recommending improvements.
      • Participate in evaluating program outcomes and impact.
      • Data collection and synthesis

    Administrative Responsibilities

    Attendance is required at:

      • Monthly Grant Advisory Committee meetings
      • Major fundraising and community events
      • Staff meetings and organizational planning sessions
      • Board meetings, as requested
      • Perform other duties as assigned.

    Qualifications

    Required

      • Master's degree in Social Work (MSW) from an accredited program.
      • Minimum of five years of experience working with children and families in a healthcare, medical social work, pediatric, behavioral health, or nonprofit setting.
      • Strong understanding of family systems, psychosocial assessment, and the impact of childhood illness or disability on families.
      • Excellent interpersonal, communication, and relationship-building skills.
      • Exceptional organizational skills with strong attention to detail.
      • Ability to manage multiple priorities while maintaining accuracy and responsiveness.
      • Strong writing and documentation skills.
      • Proficiency in Microsoft Office (Word, Excel, Outlook, and PowerPoint) and the ability to learn database systems.
      • Ability to work both independently and collaboratively within a small team.
      • Valid driver's license, reliable transportation, and ability to travel locally.
      • Successful completion of criminal background, child abuse, and motor vehicle record clearances.

    Preferred

      • Pennsylvania social work license (LSW or eligibility for licensure).
      • Experience in grant administration.
      • Experience working with nonprofit organizations.
      • Experience with client, donor, financial, or other database systems.
