    Education, and/or Work Experience Requirements:

    It is essential to have an outgoing and friendly personality with a “can-do”, “will-do” outlook. Ability to adapt easily and have a non-discriminatory nature. Employee must have strong interpersonal and organizational skills as well as a strong practical nature. The ability to write very clear and concise case notes and be computer proficient is essential. A Bachelor’s Degree in Human Services/Social Work and experience working with persons in crisis is preferred.

    Knowledge of Social Services processes including Mandated Reporting, Preventative Services and Family Development as well as other community resources and/or counseling/social work practices with high-risk populations is essential. Assisting families to achieve self-sufficiency through total “wrap around services” such as: financial assistance, food, energy reduction, Weatherization, health insurance applications, and case management is required.

      • Exceptional documentation skills.
      • Excellent written and verbal communication skills, ability to establish a rapport.
      • Ability to motivate others towards achieving goals.
      • Ability to work independently with strong sense of focus, task-oriented, non-judgmental, open personal qualities, and a clear sense of boundaries.
      • A strong sense of and respect for confidentiality involving both clients and fellow employees.
      • Ability to work in a variety of settings with culturally diverse families and communities with the ability to be culturally sensitive and appropriate.

    Physical Requirements:

    Ability to lift 30 lbs.

    Essential Duties and Responsibilities:

      • Provide case management services to all clients including: intake assessment, benefit assessment, goal setting, long term case plan development, weekly case plan development, progress monitoring, individual money management, tenant education, advocacy and referrals.
      • Maintain a caseload of 15 families.
      • Meet regularly with clients, weekly or bi-weekly for case management services.
      • Connect families with needed and available agency and community resources. Follow-up with clients and agencies as appropriate to document use/success of referral.
      • Input accurate and complete data for all contacts with clients into agency database.
      • Ensure that all documents submitted on behalf of a client are valid.
      • Contact landlords to validate eviction notices.
      • Conduct outreach to neighborhoods.
      • Provide all required documentation in a timely manner, which may include client follow-up, outcome evaluation, client contact sheets, and evaluations.
      • Participate in regular staff meetings, staff training programs, supervisory sessions, and accept the responsibility for aiding the development of positive team relationships as requested.
      • Adhere to agency policy, procedures and the agency code of ethics.
      • Other duties as assigned by supervisor.
