Job Description
The General Case Manager provides personalized case management services to homeless individuals, focusing on developing and implementing individualized housing service plans that promote housing retention and client stability. This role involves enrolling participants in the HMIS system, coordinating with landlords and housing agencies, advocating for clients in housing and benefits applications, and working closely with program staff to support a client-centered service approach.
Duties and Responsibilities
- Assess client needs and develop individualized service plans
- Enroll and maintain client data in HMIS; support housing counseling and coordinate moves to permanent housing
- Maintain accurate client records, including case notes and reports, following federal, state, county, and agency guidelines
- Monitor client progress and update service plans weekly during interim housing program enrollment
- Research, maintain, and update resources for client referrals
- Participate in community, coalition, and committee meetings as assigned
- Collaborate with Ascencia staff and external providers to ensure effective service delivery
- Assist clients with housing applications, paperwork for subsidized housing, and advocate with landlords
- Maintain professional boundaries and ethics in all client interactions
Qualifications and Requirements
- Bachelor’s degree in human services or a related field, or equivalent education and experience
- Experience in social services with knowledge of case management systems and planning techniques
- Familiarity with homeless and housing resources, housing rights, Coordinated Entry System, and Continuum of Care preferred
- Experience working with homeless individuals and families preferred
- Proficient in Microsoft Word and Excel; HMIS experience preferred
- Valid California driver’s license, active auto insurance, and ability to meet insurance carrier requirements to drive agency vehicles
- Own transportation for job-related travel
Physical Requirements
- Ability to travel to field locations as needed
- Frequent sitting, standing, bending, and stretching
- Occasional lifting of up to 25 pounds
