Job Description
The Outreach Case Manager is responsible for connecting with and engaging individuals experiencing homelessness throughout the greater Glendale area. This role focuses on building trusting relationships with people living in outdoor locations or other places not meant for human habitation and linking them to services, resources, and housing opportunities.
In addition to conducting outreach in commonly known areas where individuals experiencing homelessness congregate, the Outreach Case Manager responds to requests for assistance from first responders, business owners, residents, and hospital discharge planners. The position is based at Ascencia’s Access Center and works collaboratively with Access Center staff to ensure that client needs are addressed and individuals are connected to appropriate services and housing resources.
The Outreach Case Manager reports to the Lead Outreach Case Manager and plays a critical role in helping individuals transition from homelessness to stability by providing engagement, support, and case management coordination.
Duties and Responsibilities
Outreach & Engagement
Develop professional and supportive relationships with individuals experiencing homelessness who are staying in outdoor locations or places not intended for human habitation in the greater Glendale area
Conduct regular outreach in areas where individuals experiencing homelessness are known to congregate
Identify and document client needs through repeated engagement and contact
Provide information about available services and assist individuals with accessing transportation to service providers when needed
Respond to requests for outreach assistance from first responders, business owners, residents, and hospital discharge planners
Educate community and faith-based organizations about the work of the Outreach Team and how to refer individuals experiencing homelessness for services
Case Management Support
Work collaboratively with the primary Access Center case manager to help clients progress toward their individualized case plan goals
Monitor and evaluate each client’s progress and assist with adjusting case plans when necessary
Provide support and guidance to help clients access housing opportunities, supportive services, and other resources
Conduct crisis intervention as needed to stabilize clients and connect them with appropriate services
Documentation & Reporting
Maintain accurate and timely documentation of client contacts, service objectives, and outcomes in accordance with Federal, State, County, and Ascencia guidelines
Maintain administrative record keeping and reporting systems related to outreach activities and case management
Prepare case management related reports including program outcomes, client progress, and program successes
Adhere to strict professional boundaries and ethical standards when working with clients
Collaboration
Actively participate in client- and program-related meetings, trainings, and case conferences
Attend community, coalition, and committee meetings related to homelessness services
Coordinate with internal teams and external service providers to ensure clients receive comprehensive support
Qualifications and Requirements
Bachelor’s degree in Human Services or a related field, or a comparable combination of education and relevant work experience
Experience in a social service setting with knowledge of case management systems and planning techniques
Knowledge of outreach techniques, community resources, and the Continuum of Care for individuals experiencing homelessness
Experience working with individuals and families experiencing homelessness strongly preferred
Proficiency in Microsoft Word and Excel and ability to learn and utilize the Homeless Management Information System (HMIS)
Valid driver’s license and ability to meet insurance carrier requirements for driving approval
Must have reliable transportation to conduct job-related travel and be able to drive agency vehicles
Drug screening and TB test required prior to employment
Salary: $22 per hour
Physical Requirements
Travel to field locations required to conduct outreach and provide services
Prolonged sitting, standing, bending, stooping, and stretching
Occasional lifting of up to 25 pounds
Frequent typing and computer use, including use of keyboards and phones
