    Hours: 35 hours per week, schedules (in person or in the field)

    Office Hours: Office is open from 9:00 am to 5:00 pm, Monday to Friday

    Salary: Annualized at $57,914.57 with a comprehensive benefits package

    those we serve to live independently within their communities.

    Program Description:

    Geel Union Avenue H.D.F.C., is a four story, non-elevated building located in Community Board 2 in

    the Longwood section of the Bronx, New York. The property is conveniently located to public

    transportation; IRT numbers 2, 5 and 6 subway lines and the numbers 4, 17 and 19 bus lines. The

    single site property is comprised of twenty-nine (29) studio units. In addition to the residential

    areas, the facility has one (1) multi-purpose room (meetings, recreation, and dining) with a small

    community kitchen. On the first floor there are two (2) administrative offices (Program Director

    and Senior Case Manager) security/reception office. The lower level has access to the patio, and

    garden in the rear of building, laundry room and social service staff offices. The property has

    24/7 on site security. Residents live independently and each unit comes fully furnished. Tenants

    contribute to their subsidized rent and have access to on- site case management services.

    Essential Responsibility and Role of a Case Manager:

    • Carry a caseload of 10 to 15 cases
    • Provide case management with supportive counseling and social services with a minimum of two, 45- minute documented face-to-face contacts per month
    • Coordinate and provide instructions and information, both individual and group modalities, in the skills of daily living.
    • Coordinate and monitor that the tenant is receiving the appropriate medical, dental and mental health services and if the tenant is on medication, be able to monitor medication compliance
    • Coordinate and conduct socialization and recreational activities and groups that foster increased social skills, decrease isolation and improve cohesion and the development of a social network
    • Complete assessments and make referrals for vocational/ educational, tobacco dependency programs and substance abuse services and provide follow-up when necessary
    • Responsible for maintaining complete, accurate, and timely case records
    • Serve as an advocate/ confidante assisting tenants in obtaining services which they require and to which they are entitled. This may involve personally escorting tenants to service providers
    • Provide availability (on rotating basis) for crisis intervention and cell phone responsibility during non- scheduled work hours

    The objective of case management is the continuity of care and service to each tenant/client while

    meeting the standards, philosophy, and policies of Geel Community Services in accordance with NYC

    Dept. of Social Services, DSS and NYC Department of Health and Mental Hygiene (DOHMH) guidelines.

    Qualifications and Requirements:

    • Education: Bachelor’s degree in Social Work or a related human services field is required
    • Experience: The preference is that the applicant have at least one year’s experience in the field of social services. Bi-lingual Spanish a plus

    Physical Requirements:

    • Must be able to access areas from the basement to the roof in a non-elevator four story building
    • Must be able to travel to different locations utilizing public transportation
    • Must be able to demonstrate proper cleaning and apartment maintenance methods to residents in addressing their activities of daily living.
