Established in 1993, Community Health Partnership advocates for affordable, accessible, and culturally competent health care systems that demonstrate respect and compassion for our diverse communities. Originally formed as a response to managed care, we have evolved to grow in five strategic areas: Advocacy, Community Health, Workforce Development, Population Health, and Community Engagement. Today, CHP represents 10 organizations with a total of 40 sites that annually serve about 275,000 patients.
