LHM Foundation’s transitional housing program – Horizons – will provide a critical resource for individuals and families facing homelessness in South Central Pennsylvania. Horizons will offer a supportive residential community within our family care facility, as well as individualized services and supportive programming tailored to help participants overcome their barriers to housing stability.

There is a transitional housing shortage across our local communities. As a result, many in need go without supportive programming to help them achieve housing security. Through a comprehensive approach that addresses immediate needs and provides the skills and resources to empower long-term stability, the Horizons program will focus on creating bright, sustainable futures for its participants.
