Partners for Health Foundation
Partners for Health Foundation’s vision is to make our 15 communities healthier, better places to live. Since becoming an independent public charity in 2008, we have awarded more than $7 million and partnered with more than 60 organizations, congregations, school districts and municipalities to make progress in key areas. The Foundation focuses on harnessing the efforts of engaged partners to work collectively toward shared goals, such as addressing the growing problem of hunger and homelessness. We support efforts to make healthy choices easier by supporting policy, system and environmental changes that promote healthy people in healthy places. We work to connect older residents who are at greatest risk emotionally, physically and economically with services and activities, and opportunities to engage in civic life. And we support mental health, which plays a major role in people’s ability to maintain good physical health.
