    About Us

    Center for the Heart (C4TH) is a nonprofit organization dedicated to making compassionate, grief-informed end-of-life care universally accessible. We serve individuals and families navigating Alzheimer’s, dementia, terminal illness, and complex caregiving experiences—regardless of income, background, or life circumstance. Our mission is to ensure that every person can meet the end of life with dignity, clarity, and connection, and that no one has to walk through death or grief alone.

    Our work centers on three pillars: direct support, education, and community awareness. Through our end-of-life doula services, we provide emotional, practical, and relational support—ranging from comfort-care planning and vigil presence to legacy work and family guidance. Our grief services extend care beyond the moment of loss, offering both individual and group support for adults, families, and organizations. We also train caregivers, professionals, schools, and community partners to build death- and grief-literacy, equipping them with the skills and understanding needed to support others with confidence and compassion.

    C4TH currently serves communities across New Jersey, New York, and Pennsylvania through in-person care, and extends support nationwide through virtual services. Rooted in compassion, equity, and holistic care, we are committed to transforming how individuals, families, and communities experience end-of-life and grief—so that every journey is met with honor, dignity, and human connection.
