    To ensure the physical, emotional, social, and mental well-being of adolescents and young adults served by NYC Health + Hospitals as they transition from childhood into adulthood.     The NYC Health + Hospitals Adolescent Health Program (AHP) has been working since 2012 to improve the health and development of adolescents and young adults across New York City. This YouthHealth program is funded by the Young Men’s Initiative through the Mayor’s Office for Economic Opportunity/Children’s Cabinet and is dedicated to making system-wide changes to achieve its goal.
