Community Health Improvement Partners (CHIP) strives to be the established leader in orchestrating collaborative solutions to address critical community health issues in the San Diego region. We envision communities where everyone has the opportunity to achieve optimal health and well-being. We achieve this through our mission to advance long-term solutions to priority health needs through collaboration and community engagement.
Based on a rigorous community health needs assessment process, CHIP’s current community health priorities are obesity, mental health, violence and injury prevention, access to care and social determinants of health. Our major programs and initiatives include the San Diego County Childhood Obesity Initiative, Resident Leadership Academy, Suicide Prevention Council, Independent Living Association, Kaiser Permanente Healthy Eating Active Living Zone Project - Lemon Grove, Racial and Ethnic Approaches to Community Health Project - Chula Vista, Nutrition and Healthcare Leadership Team, Farm to School Task Force, and Health Literacy San Diego.
