About Us
The Root Cause Coalition, a non-profit, member-driven coalition, was established in 2015 to address the root causes of health disparities by focusing on their social determinants. Having grown to over 30 partners representing healthcare providers and payers, nonprofit foundations, community service organizations, educational institutions, and professional associations, we work to reverse the epidemic of preventable chronic health conditions through education, research and advocacy.
With solid engagement of the healthcare community, our vision is for The Root Cause Coalition to be the leading organization in addressing the social determinant causes of poor health outcomes at local, state and national levels with the ultimate goal of improving the lives of individuals and communities across the country.
