    Horizon Health and Wellness is a non-profit 501(c)(3) integrated healthcare agency licensed by the State of Arizona to provide an array of outpatient and residential services in Pinal, Gila, and Yuma Counties as well as Habilitation services in the counties of Cochise, La Paz, Maricopa, and Santa Cruz. The Agency has been in operation for over 30 years providing quality behavioral health services in the identified geographical service areas. Our innovative approach to addressing the whole person's wellbeing is helping to change the lives of the underserved in Arizona.

    About Us

    We provide integrated healthcare services including crisis support to adults with serious mental illness, children, families and also individuals recovering from substance abuse or other serious issues.

    We have group homes for developmentally disabled, a domestic violence safe home and an assisted living facility. We offer employment services and housing opportunities.

    We have two inpatient facilities and a 23 hour observation unit. We also provide primary care to the communities we serve.
