Over the past four decades, we have become a leading expert in the delivery of person-centered, community-based care coordination services and population health. With a rich history of innovation, The Center has translated national movements into ground-level, community-based programs including Medicaid rebalancing initiatives, aging in place, transitions of care, population health, and housing first. In addition, we are advocates for policy and system changes that positively affect the lives of people with the most complex needs.
