Pathfinders For Hope

Pathfinders for Hope is a Vallejo, California nonprofit building Operation Iron Gate, a closed-loop referral accountability model. A referral stays open until the receiving organization confirms it or records why it cannot proceed. It works alongside existing HMIS and case systems. Pathfinders for Hope brings lived experience, community engagement, and system design together to improve how organizations coordinate referrals. Operation Iron Gate is being developed with nonprofits, agencies, recovery providers, county social services, domestic violence organizations, and city and county partners. The Vallejo pilot is not open for public intake. Pathfinders is currently recruiting community-resource and pilot partners.
