Summit Community Care Clinic

The mission of the Summit Community Care Clinic (Care Clinic) is to provide exceptional, integrated, patient-centered health care services designed to meet the needs of people who experience barriers to accessing care, regardless of their ability to pay. The Care Clinic has been operating as a Federally Qualified Health Center facility since 2015, originally starting as a safety net clinic in 1993 and becoming a Non Profit in 2003.

Summit Community Care Clinic located in Frisco Colorado with School Based Health Clinic in Lake County and Summit Schools.  In 2015, the Care Clinic served over 7,000 patients with 16,000 primary, dental and behavioral health visits.   Patients are primarily from Summit County but there are a growing number of patients seeking services from Eagle, Lake, Grand Park and Clear Creek Counties. 

Summit & Lake School District – School Based Health Center provide quality primary care, oral health and behavioral health services to Summit and Lake County students and staff at five sites in Summit Schools through school based health centers. School Based Health Centers bring the doctor’s office to the school. The clinics are located in Dillon Valley Elementary (DVE), Silverthorne Elementary (SVE), Summit Middle School (SMS) and Summit High School (SHS) and Lake County Middle/High School.
