The staff and volunteers of Providence Community Health Napa Valley are committed to providing exceptional professional health care, expert resources and compassionate support to individuals and their families experiencing the transitions of aging, learning to live with chronic illness or facing the end of life.

We have been dedicated to serving the needs of Napa Valley for close to four decades. Formerly Collabria Care our new name reflects our connection to Providence Health Care System and the continuum of care that we provide for those experiencing the transitions of aging. We are an Alzheimer's Resource Center, Palliative Care Provider, Information and Assistance, in addition to providing outstanding hospice care and recently developing the PACE program which is an all inclusive health care plan for people over 55 with skilled care needs, living safely in the community. Income from our Thrift and Resale Stores support our programs. Volunteers are utilized fully in all programs.
