About Us
Healthy Philadelphia is a non-profit organization committing to making health care more accessible and high quality for all Philadelphians by engaging the community and stakeholders. Healthy Philadelphia was founded in response to a ballot referendum in Philadelphia calling for universal access to decent health care.
Healthy Philadelphia is currently leading two major initiatives. First, Healthy Philadelphia is working to improve primary care access in Philadelphia neighborhoods experiencing critical access problems with a current emphasis on Northeast Philadelphia. Second, Healthy Philadelphia is convening hospitals and community health centers to improve transitions of care for vulnerable patients.
