The Health Equity Partnership (CHNA9) is a broad coalition of residents, health and human service organizations, education institutions, private businesses, foundations, governmental bodies, and community groups working to promote health equity in the communities of North Central Mass. CHNA 9 holds the regional Community Health Improvement Plan (CHIP). This plan aims to address identified community health priority areas through policy advocacy, systems change and development, and community engagement and empowerment. Communities of North Central Mass include Ashburnham, Ashby, Ayer, Barre, Berlin, Bolton, Clinton, Fitchburg, Gardner, Groton, Hardwick, Harvard, Hubbardston, Lancaster, Leominster, Lunenburg, New Braintree, Oakham, Pepperell, Princeton, Rutland, Shirley, Sterling, Templeton, Townsend, Westminster, and Winchendon.
