  • About Us

    With over 800 participant organizations and more than 3,000 clinical providers (including 1,100 PCP’s), the Community Care of Brooklyn (CCB) performing provider system (PPS) aims to create a new model of coordinated care for Brooklyn’s Medicaid population. CCB includes organizations from across the social and clinical service continuum, including Interfaith, Kingsbrook Jewish, Maimonides, New York Community, New York Methodist, and Wyckoff as Brooklyn hospital partners.

    CCB will use DSRIP funding to further leverage clinical and technological infrastructure developed collaboratively over the last five years to establish an integrated system of care across a broad range of medical, behavioral and social sectors. This will improve care coordination and communication among providers throughout the borough and enhance population health management for the most complex patients.
