Our Vision is to empower the disenfranchised by building self-esteem, self-worth and promoting responsible and healthy lifestyle choices, ultimately creating self-reliance. Our Mission is to provide support, education and resources that promote healthy lifestyles, decrease health disparities and increase access to quality health care.
About Us
Heart to Hand, Inc. provides prevention and intervention services to individuals, families infected/affected by HIV/AIDS. H2H also decreases health disparities by providing navigation to uninsured or underinsured individuals in order to improve access to quality health care. We meet people where they are and "extend a touch of love."
