About the PACE Organization of Rhode Island:

With a mission to preserve and sustain the independence of older adults (55 and older) who have significant health needs and wish to remain in the community, the PACE Organization of Rhode Island provides community-based health care and support services in order to ensure the needs of our participants are fully met through a plan of care that is specific to each and every person.

PACE-RI is a unique model of care which brings together a coordinated team of doctors, nurses, social workers and therapists to provide care that addresses the specific medical and social needs of older adults. The result is higher quality care and far more favorable outcomes compared to traditional care options. On average PACE-RI keeps participants out of a nursing home for four years.
