Augustana Care's mission is to serve God by fostering fullness of life to the elderly and others in need through the provision of health care, housing, and other services in a Christian tradition.
About Us
Augustana Care Health & Rehabilitation of Apple Valley provides services for those needing long term and short term rehabilitative care. We have a 56 bed transitional care unit and 122 beds for long term skilled nursing care. Our goal is to help each individual live to their maximum potential through rehabilitative therapy, nursing care, and activity programming that fosters fullness of life.
