About Us

Established in 1979, Community Hospice of Northeast Florida is the first and only locally based, nonprofit hospice program to serve residents of the First Coast. Each day, the organization helps approximately 1,300 patients and their families live better with advanced illness. During its 34-year history, Community Hospice has served more than 85,000 adults and children throughout Baker, Clay, Duval, Nassau and St. Johns counties—at home, long term care and assisted living facilities, in hospitals and its six inpatient care centers.

Community Hospice ensures patients' comfort by addressing their physical, emotional and spiritual needs through an interdisciplinary team of 940 hospice professionals and 930 volunteers. The organization also assists family caregivers through education, access to community resources, and bereavement support for at least 13 months after a loss.

Community Hospice Foundation SM generates philanthropic and community support for patient care services, family needs and community programs, such as Community PedsCare®, a nationally recognized pediatric palliative and hospice program.

The organization is committed to improving the quality of life for patients and their families and to be the compassionate guide for end-of-life care in our community.
