The Southwestern Connecticut Agency on Aging (SWCAA) is where you turn when you need unbiased, accurate information to help meet the challenges of aging and disability in the community. Federal and State funding supports SWCAA’s information specialists which in turn allows staff to refer based on what you need rather than who will profit. Many of SWCAA’s services are provided free or at sliding scale and our information line, 800-994-9422 is available to everyone at no charge. The Southwestern Connecticut Agency on Aging (SWCAA) is a private, not-for-profit agency, established in 1974. SWCAA receives funds through the Older Americans Act and works with dedicated volunteers to award grant funds to community organizations serving older adults. Grants support services such as home delivered meals, in-home care, legal, health, adult day care, transportation, senior centers, outreach and social support.
SWCAA conducts a needs assessment and offers community education, information and referral in support of older persons and persons with disabilities. The Southwestern CT Agency on Aging helps older adults and persons with disabilities make informed choices about their health, insurance, housing and long term care.
The Agency supports family caregivers by providing temporary care and assistance for eligible persons designed to give caregivers a much needed rest. SWCAA is also an “Access Agency”, under contract with the State of Connecticut, Department of Social Services, to provide care management to Medicaid clients who prefer to receive home and community-based services rather than institutional care. SWCAA’s care managers are social service professionals who help assess and build a person-centered plan of care designed to help elders, persons with disabilities and persons with acquired brain injury.
