Neighborhood HealthSource is a Federally Qualified Health Center, operating four primary care clinics in the neighborhoods of North and Northeast Minneapolis for more than 45 years. Our mission is to "improve the health of our communities by providing quality health care services that are affordable and accessible." Clinic services include basic and preventive health care for adults and kids, screenings and immunizations, minor procedures, chronic disease management, mental health counseling and maternity and newborn care. NHS also provides various community health/outreach programs, focused on childhood obesity, stroke, diabetes and breast cancer prevention, family planning and STDs.
