Chosen Hearts is a South Los Angeles nonprofit dedicated to rebuilding lives and restoring hope for individuals and youth facing homelessness, housing instability, and systemic barriers.
We provide trauma-informed Case Management, Housing Stability & Tenancy Support, Independent Living Skills coaching, Life Management programming, youth wellness initiatives, and community-based outreach designed to promote long-term stability and self-sufficiency.
Our work includes four core service areas:
Case Management
We offer individualized, one-on-one support focused on stabilization, comprehensive needs assessment, care coordination, barrier reduction, and connection to housing, benefits, mental health care, and community resources. Our approach is trauma-informed, client-centered, and grounded in accountability and empowerment.
Housing Stability & Tenancy Support
We support individuals transitioning from homelessness into permanent housing through tenancy education, lease understanding, budgeting support, landlord communication guidance, routine-building, and problem-solving. Our goal is to prevent returns to homelessness by strengthening housing retention and long-term stability.
Independent Living Skills & Life Management
Through structured coaching and guided goal-setting, participants build practical life skills, strengthen decision-making, improve financial management, and develop confidence to navigate major life transitions successfully.
Rooted & Resilient: A TAY Health & Healing Series
This youth-centered initiative serves Transitional Age Youth (ages 16–24) in SPA 6 through trauma-informed wellness circles and health education that promote resilience, emotional wellness, and community connection.
We meet people where they are — in schools, shelters, interim housing, and community spaces — providing consistent, dignity-centered support.
Since launching, Chosen Hearts has served over 1,000 individuals through outreach, stabilization services, and structured programming.
