    The Cook County Health Foundation (CCHF) is a 501(c)(3) Illinois non-profit founded to advocate and raise funds for programs and services that improve quality patient care and meet the health care needs of the communities served by the Cook County Health & Hospital System (CCHHS).

    CCHHS has been serving the greater Chicago area since 1835 and has led the way in providing pioneering care while playing the critical role of ensuring access for all residents. The Foundation, formed in 2011, supplements government funding provided to the System and is governed by an independent board of directors.
