Description
The job description for a Women's Intake Specialist includes the following responsibilities:
- Coordinated Intake and Referral (CI&R): The primary point of entry for various community home visiting, care coordination, and support services.
- Client Assessment: Contacting, assessing, and providing brief education and information to clients.
- Referral Process: Assisting clients in selecting a home visitation service based on needs and preferences.
- Program Eligibility: Determining and designating a Program Eligibility based on client information.
- Community Resources: Providing referral information to educate and improve client knowledge and understanding of available community resources.
- Client Records: Ensuring that all client records are accurate, current, and updated on a regular basis.
- HIPAA Compliance: Observing HIPAA guidelines and maintaining the confidentiality of information.
