Description
Diana Harbor & Haven Foundation is seeking two compassionate, dedicated Case Managers to join our growing nonprofit organization.
Our mission is to provide safe, supportive housing and pathways to independence for individuals facing housing instability, including returning citizens, seniors, and youth transitioning out of foster care.
Key Responsibilities:
- Develop individualized service and stability plans.
- Connect residents with employment, healthcare, and community resources.
- Monitor resident progress and maintain case documentation.
- Advocate for residents and coordinate with community partners.
- Support residents in achieving long-term independence.
Ideal Candidates:
- Background in social work, human services, psychology, or related fields.
- Experience working with vulnerable populations.
- Strong communication, organizational, and problem-solving skills.
- Passion for helping individuals rebuild their lives.
