Hospice and Transitions Companion Volunteer

• POSITION SUMMARY: The Hospice and Transitions Volunteer Companion Volunteer is an additional member of the Hospice interdisciplinary team, working with patients/families, and clients/families in conjunction with other members of the team. They provides non-medical support services as indicated by the Hospice or Transitions plan of care, within the policies and procedures of the JSSA Hospice Department. These services generally fall under areas of practical assistance; in person companionship and emotional support; in person respite care; in person live music; in person arts and crafts; life review; reading; errand and transportation support; and companion phone calls.

  TIME REQUIRED: Minimum of 2 hours per month, weekdays or weekends, days or evenings

  CLIENTS: JSSA Hospice patients and JSSA Transitions clients

  Frequency typically 2 times per month minimum
