    At Transitions, we believe that exceptional care extends far beyond medical visits. For military veterans navigating life's final journey, having a fellow veteran or dedicated advocate visit can ease isolation, honor their service, and bring profound emotional comfort.

    Through our Veteran Companionship Volunteer program, you are matched with a veteran patient in your immediate local area to offer unique peer support, connection, and respect. Whether you are a veteran yourself wanting to support a brother or sister in arms, a military family member, or a compassionate neighbor honoring those who served, your presence is the greatest gift you can offer.
