Description
Purpose: Enhance integrated psychotherapy services to patients at Phoenixville Free Clinic.
Provide individual, couples, and/or family consultations and psychotherapy with patients including:
Comprehensive biopsychosocial assessments and baseline screenings and corresponding diagnosis.
Basic psychoeducation for patients and their support systems.
Medication adherence and disease self-management counseling.
Motivational interviewing and other therapeutic techniques to develop behavioral strategies aimed at symptom reduction and improved quality of life.
Self-care plan development and problem-solving skills to facilitate disease self-management, improved coping, distress tolerance, stress reduction, and relaxation.
Evaluation and identification of maladaptive coping strategies and development of harm reduction strategies.
Follow-up with patients and coordinate services with other providers (internal and external) as needed.
Develop treatment goals and discharge plans with formal and informal recommendations and referrals.
Complete all charting as needed and required, including assessment, diagnosing, and progress notes.
Participate in continuing education and professional development to stay up-to-date with the latest research and treatments to provide the best care possible
Adhere to all healthcare laws, regulations, and standards to ensure patient safety and confidentiality.
