Description

Location: Remote
Hours: 20–25 per week
Position Overview
Support participants directly through intake, assessment, referrals, and follow-up. Ensure participants are connected to services and support, and maintain accurate program records.
Responsibilities
• Conduct participant intake and assessments
• Develop individualized support plans
• Coordinate referrals and follow-ups
• Maintain participant records and reporting
• Support Program Managers in service delivery
Qualifications
• Experience in case management, social work, or nonprofit service delivery
• Strong interpersonal and communication skills
• Organized and detail-oriented
• Comfortable managing multiple participants and tasks simultaneously
