    Description

    Veteran to Veteran Connection Volunteer

    Works With: Patients who identify as Veterans

    Role

    Veterans support veterans on an end-of-life journey. This role provides veteran patients with support through compassionate presence and socialization. From simply being present,to reminiscing, even reading the paper, it's a way for hospice veterans to be connected to other veterans.

    Primary Responsibilities:

    Compassionate presence for the patient and family

    Support patient on their end of life journey as needed

    Other services as determined by volunteer, coordinator and patient/family

    Secondary Responsibilities

    Simple documentation of the visit, form provided, to be submitted to the Volunteer Coordinator

    Desired Experience

    A successful applicant will show a positive attitude towards patient care and be comfortable around individuals on their end of life journey. Must be a Veteran.
