    Description

    Make calls to check in on our patients and families. Care Calls are made every Thursday to our home patients/families. We check in to see if they are comfortable, need any supplies or just want us to get a message to their care team! This service allows us to deliver medication and supplies on Friday’s, so families don’t have to worry about running out on the weekend. These calls can be made from our office or your home! We have a simple script and will train and support you!
