We are seeking creative and compassionate volunteers to participate in our Chart‑A‑Life program, a meaningful hospice initiative that honors the life stories of individuals receiving care. Volunteers help create personalized visual legacy boards that celebrate a person’s history, relationships, and cherished memories.
This unique opportunity allows you to bring comfort, dignity, and connection to patients and their families during a deeply important time.
What You’ll Do
As a Chart‑A‑Life Volunteer, you will:
- Collaborate with hospice staff and caregivers to gather personal stories and meaningful details
- Use a guided worksheet to learn about the individual’s life, milestones, relationships, and interests
- Create a visual memory board or scrapbook-style display using photos, quotes, and keepsakes
- Assemble and deliver the completed “Chart‑A‑Life” to the care team or family
- Help spark reminiscence, sensory engagement, and joyful moments for patients
The information used to create each board may include:
- Life history, career, and important milestones
- Family, friendships, and meaningful relationships
- Hobbies, favorite activities, and special memories
- Traditions, favorite foods, music, and meaningful quotes
