    Position Overview

    Human Health Project (HHP) is seeking a proactive and community-focused Los Angeles Community Partner Coordinator to strengthen HHP’s relationship with the 211 information and referral network.

    211 is a public information line that connects individuals and families with health, social, and community services. It serves as a non-emergency counterpart to 911 and 311 and is available throughout California and much of the United States.

    HHP is currently included in the 211 resource directory, but the organization’s listing is incomplete and does not accurately communicate the services and resources HHP provides. This role will take ownership of improving HHP’s presence within the Los Angeles 211 system, establishing an ongoing relationship with relevant 211 staff, and developing a strategy for expanding HHP’s visibility across California.

    Candidates must be within LA County, or be able to access the San Gabriel area. This is a hybrid position.

    The ideal candidate is organized, persistent, comfortable communicating with external organizations, and motivated by improving public access to health information and support services.

    Key Responsibilities

    • Review HHP’s current listing within the Los Angeles 211 resource directory and identify missing, inaccurate, or unclear information.
    • Coordinate with HHP’s Program Team to develop an accurate and accessible description of HHP’s services, eligibility requirements, referral processes, and contact information.
    • Serve as HHP’s primary point of contact with 211 Los Angeles and related information-and-referral organizations.
    • Contact 211 representatives to understand listing requirements, approval procedures, update processes, and opportunities for greater organizational visibility.
    • Submit and follow up on listing updates until HHP’s information is accurately reflected in the 211 system.
    • Build and maintain a professional relationship with 211 staff and community resource specialists.
    • Create an internal process for regularly reviewing and updating HHP’s 211 information.
    • Research how 211 services are structured across other California counties and regions.
    • Develop a practical expansion plan for increasing HHP’s presence throughout California’s 211 network.
    • Identify additional referral networks, community directories, healthcare organizations, and social-service partners that could connect individuals with HHP.
    • Maintain clear records of outreach, contacts, submissions, follow-up dates, outcomes, and next steps.
    • Provide regular progress updates to HHP’s Program Team and collaborate with Business Development and other departments when appropriate.

    Initial Priorities

    During the first phase of the role, the coordinator will be expected to:

    1. Audit HHP’s existing Los Angeles 211 listing.
    2. Confirm the correct process for revising and expanding the listing.
    3. Work with the Program Team to prepare accurate program descriptions and referral information.
    4. Establish direct communication with the appropriate 211 representatives.
    5. Complete and verify the updated Los Angeles listing.
    6. Develop recommendations for expanding HHP’s presence across California.

    Qualifications

    • Strong written and verbal communication skills.
    • Excellent organization, documentation, and follow-up abilities.
    • Comfortable conducting professional outreach by email, phone, and virtual meetings.
    • Ability to independently manage a project from research through implementation.
    • Strong attention to detail, particularly when reviewing public-facing information.
    • Interest in public health, community health, healthcare access, social services, patient advocacy, or nonprofit work.
    • Ability to collaborate across multiple teams while maintaining clear ownership of assigned responsibilities.
    • Persistence and professionalism when navigating administrative systems and external organizations.

    Preferred Qualifications

    • Experience in community outreach, partnership development, program coordination, public health, social services, healthcare navigation, or nonprofit operations.
    • Familiarity with 211 systems, community resource directories, referral networks, or information-and-referral services.
    • Knowledge of Los Angeles County health and social-service organizations.
    • Experience working with underserved or medically vulnerable communities.
    • Experience developing partnerships across multiple counties or geographic regions.
    • Spanish, Korean, or another language commonly spoken in Los Angeles communities is a plus.

    Success in This Role

    Success will be measured by:

    • Completion of an accurate and comprehensive HHP listing within the Los Angeles 211 system.
    • Establishment of a reliable working relationship with relevant 211 contacts.
    • Creation of a documented process for maintaining HHP’s listing.
    • Development of a clear strategy for expanding HHP’s presence across California.
    • Growth in referral partnerships and public awareness of HHP’s resources.
