    Description

    Make Check-in Calls to Patient/Family based on your availability!

    The Circle of Care program at AccentCare Hospice provides regular phone calls to our home patients. The goal is to assess their symptom management and meet the needs for medications and supplies before the weekend.

    It is also an opportunity to assess the quality of the care our teams are providing. These calls are

    typically completed on Wednesdays or Thursdays; think of them as a “tuck in” call before the weekend.

    We aim to assign each volunteer to the same patients and their families/chosen families each week so that it’s a friendly, familiar voice, and the calls become another layer of virtual support. Any needs identified are relayed to the hospice leadership team so they can be addressed in a timely manner.
